Appointments:

As of mid September 2019, Moshe will be practicing Sunday through Thursdays and will meet clients exclusively via tele-conference using a HIPAA approved platform.

EMERGENCIES:

In the case of an emergency, please be conservative and either call 911, go to an emergency room or call the Georgia Crisis Access Line at 1-800-715-4225.

INSURANCE:

Moshe is in network with Blue Cross/Blue Shield, Aetna and Tricare. Please contact your insurance company prior to the first appointment to understand your benefits inlcuding:

1. Your obligation for any annual deductible

2. Your obligation for any per session co-payment

3. The limits of your coverage

4 The need for any pre-certifications for treatment.

Payment: As Moshe works remotely, credit , debit and HSA cards are accepted. With your permission he will record your account information for greater ease in billing.

There is a pro-rated fee for phone and collateral consultations of $175/45 minutes. Insurance cannot be billed for such consultations.  

CANCELLATION POLICY:

There is a 24 hour cancellation requirement for both therapy and supervision appointments. Cancellations of less than 24 hours will require payment of the full fee that would have been collected from the client and/or insurance company for that session.

Hours:

By appointment only, days and evenings. Late afternoon and evening hours are often full, morning and afternoon times are often more available.
